$40,000 - Information, referrals/care coordination, bereavement help for kids and their families

hen a child has a life-threatening illness, the stress, anxiety, fear, and uncertainty profoundly affect the entire family. As they face difficult decisions about their child’s treatment and care, parents often feel overwhelmed with feelings that there is no where to turn for help. This funding helps Jacob’s Heart bridge these gaps for families, helping them to access information, support and counseling services.  Jacob's Heart supports families from the time of a child's diagnosis with cancer through treatment and, if the disease is incurable, through the child's death and beyond.  This grant helps fund Jacob's Heart care coordination services, as well as emotional and bereavement support to these families.  HCP professionals provide a key linkage for families struggling to cope with overwhelming anxiety, stress, depression and other emotions surrounding the serious illness or loss of their child.
