$100,000 - Care coordination, emotional support, referrals and information, advocacy

he prospect of the loss of a child to a life-threatening illness is so difficult for families that navigating through the choices about care, services, specialties, medications, as well as dealing with their fears and anxieties is more than most families can often bear. They are often overwhelmed by it all. This grant helps fund CHPCC's program to help families in their communications with health care providers and others, and helps them navigate through the confusing processes of insurance coverages and other issues. Staff also provides referral services and helps families streamline the process of finding the most appropriate care for their child.
